The Public Utilities Departments listed below have requirements that, if not followed, can cause problems and/or delays submitting test results.
Newport News requires the original test form (one per device) sent to each customer be used to submit the test results. A good quality copy of the original may be used. Generic forms may not be used. The original form has coded information specific to the individual device. If the form is lost, the customer must contact Newport News Public Utilities to have a replacement form sent.
Please have the form available onsite when the tester arrives. We will complete and submit the report to the proper authorities and furnish a copy to the customer by email.
